Prediction of left ventricular peak ejection velocity by preceding and prepreceding RR intervals in atrial fibrillation: a new method to adjust the influence between two intervals.
نویسندگان
چکیده
In atrial fibrillation, cardiac performance is dependent on both preceding RR (RR-1) and prepreceding RR (RR-2) intervals. However, relative contributions were not well defined. Left ventricular outflow peak ejection velocity (Vpe) was measured by echocardiography from 21 patients. The relation between RR-1 and Vpe could be divided into two zones; steep slope in short RR-1 intervals (< or =0.5 sec) and plateau in long RR-1 intervals (> 0.5 sec). RR-2 had a weak negative association with Vpe. The mean squared correlation coefficient (r2) between RR-2 and Vpe was 0.15 +/-0.13 and improved to 0.29+/-0.21 (p<0.001), when coordinates with RR-1 < or =0.5 sec were excluded. The RR-1 was positively associated with Vpe. The mean r2 between RR-1 and Vpe was 0.52+/-0.17 and improved to 0.72+/-0.11 (p<0.001), when adjusted by RR-2. Simple linear regression analysis showed that mean RR interval, age, fractional shortening (FS), and mean peak velocity were negatively correlated with modified r2 between RR-2 and Vpe. Multiple stepwise regression analysis revealed that mean RR interval (r2=0.32) and FS (r2=0.16) were significant. In summary, simple modification could improve the relationship of both RR-1 and RR-2 with cardiac performance. RR-2 might play a more role in cardiac performance than previously expected, and when cardiac function was impaired.

Left ventricular beat-to-beat performance in atrial fibrillation: contribution of Frank-Starling mechanism after short rather than long RR intervals.
OBJECTIVES This study sought to evaluate control mechanism of the varying left ventricular performance in atrial fibrillation. BACKGROUND Atrial fibrillation is characterized by a randomly irregular ventricular response, resulting in continuous variation in left ventricular beat-to-beat mechanical behavior and hemodynamic variables. METHODS Fourteen patients with chronic nonvalvular atrial ...
متن کاملNew Parameters for Left Ventricular Function in Atrial Fibrillation: Based on the Relationship between RR Interval and Performance
This study was designed to obtain new parameters representing left ventricular (LV) function independent of irregular RR intervals in atrial fibrillation (AF). AF patients were divided into Normal (n=9) and LV Dysfunction (n=9) groups. The relations between LV outflow peak ejection velocity (Vpe) and preceding (RR-1) or prepreceding RR intervals (RR-2) were obtained using logarithmic equations,...
متن کاملRelation Between RR Intervals and Early Diastolic Mitral Annular Velocities in Atrial Fibrillation
BACKGROUND AND OBJECTIVES Irregular RR intervals in atrial fibrillation (AF) make beat-to-beat changes in left ventricular (LV) systolic performance. Early diastolic mitral annular velocity (E') is one of the well-established parameters for evaluating LV diastolic function. The relation between RR intervals and E's is unknown. The aim of this study was to observe the influence of continuous cha...

INTERVENTIONS Invasive and non-invasive haemodynamic variables were assessed using a non-imaging computerised nuclear probe, a balloon...
متن کاملClinical Significance of P Wave Dispersion in Prediction of Atrial Fibrillation in Patients with Acute Myocardial Infarction
Background: P wave dispersion (PWD) is defined as the difference between the maximum P wave duration (Pmax) and the minimum P wave duration (Pmin) in 12-leads of the surface electrocardiography. The aim of this study was to evaluate the values of PWD during atrial fibrillation (AF) after acute myocardial infarction (AMI). Methods: We prospectively evaluated atrial rhythms of 350 patients (251 m...
